Some key services from Evergreen Surgery are now available online. You can book appointments, request repeat medication and view key parts of the medical record (including some test results). You don't even need a computer as there are Apple and Android compatible smartphone apps available to download free from the respective app stores. Search for "Patient Access" to download the app provided by our Clinical Record System Provider, EMIS.

What is Patient Access?

With Patient Access, you can now view, book and cancel appointments at Evergreen Surgery from home, work or on the move — wherever you can connect to the internet. What is more, because Patient Access is a 24 hour online service you can do this in your own time, day or night.


Plus, Patient Access is not just for booking GP appointments, you can also:

  • Order repeat prescriptions.
  • Change your address details.
  • View key parts of your medical record securely online (you will need to make a separate application for this for confidentiality reasons)

How do I register?


Please come to the surgery and bring with you some identification, preferably photo ID if you have it. This is necessary to protect your confidentiality and to ensure that only you have access to your information. The receptionist will give you a Personal Indentification Number (PIN). Once you have this access code, you can register here.


If you would like to have access to your medical record online, please ask for this to be set up. We are required by law to see identification to confirm your identity before we can give you access. In a very few cases, the surgery may decide to limit access if it is felt that it would be hamful to the patient concerned to grant it, but we would discuss this with you if this was the case.

Is my information secure?

All information that is sent to your surgery via Patient Access is secure. Your personal details are encrypted and protected using the highest standard internet security, so it cannot be intercepted. Only you and your GP surgery are able to see this information.
