How To Register As A New Patient


Patients wishing to register should complete the Surgery's new patient registration form, available from reception. Please ensure you complete all the questions. If you do not, it may delay your registration being accepted by the Primary Care Trust (NHS Enfield). Once completed, please bring all relevant information with you, including evidence of identity and proof of address (so we can be sure you live in our practice area). It will save time if you can bring your NHS number (on your medical card or available from your last GP) and your last doctor’s details. Examples of evidence include: Passport, Driving Licence, other photo ID, utility bills, bank/ building society statements, letters from a government agency or solicitor. Please ask at reception for details, or if you have difficulty producing any of these. All new patients are also asked to complete a comprehensive health questionnaire.


Your full medical records will be transferred from your last GP practice automatically once your registration is processed by NHS Enfield, but this process may take a few weeks. If you had repeat medication at your last surgery, a doctor here will need to see you before prescribing more. Please bring your repeat prescription form from your last surgery with you. For some medication, we may need to contact your last GP before prescribing for you.


If you are only staying in the area for a short time (three months or less - for example, if you are visiting a relative), we may be able to register you as a temporary patient. Please ask at reception for advice.
